The GUIDE (Guiding an Improved Dementia Experience) Model is an innovative program introduced by the Centers for Medicare & Medicaid Services (CMS).
This program is designed to provide comprehensive, coordinated care for individuals living with dementia. The GUIDE Model’s mission is to improve the quality of life for dementia patients, reduce the burden on their caregivers and enable individuals to continue living in their homes and communities for as long as possible.
As a participant in the GUIDE Model, We are committed to delivering high-quality care and support through personalized care plans, caregiver education, and comprehensive care coordination.
Through the GUIDE Program, we provide comprehensive, coordinated care to improve quality of life, support caregivers, and help individuals living with dementia remain at home for as long as possible.
Who is eligible for the GUIDE Program?
The GUIDE Model is for people living with dementia who meet the program's eligibility requirements. Generally, individuals must have dementia confirmed by a clinician, have Original Medicare (Parts A and B) as their primary payer, live in a private residence or an eligible residential care community, and meet other CMS requirements.
What does the program cost?

Yes. Respite services are available through the GUIDE Model for eligible participants. Respite can provide temporary relief for qualifying caregivers while supporting the person living with dementia.
